1972-LB: Glucagon-Like Peptide 1 Receptor Agonists and NAION—A Systematic Review and Meta-analysis

Introduction and Objective: Recent real-world evidence studies investigating semaglutide and a possible increased risk of nonarteritic anterior ischemic optic neuropathy (NAION) present conflicting findings. Our study aims to examine available evidence to evaluate the risk of developing NAION in semaglutide users compared to non-users. Methods: Seven retrospective studies assessing NAION risk among semaglutide users compared to non-users were identified by the authors through targeted searches and awareness of recent publications. Reported hazard ratios (HRs) from identified studies were pooled, and a forest plot was generated. Results: Our meta-analysis yielded a pooled HR of 1.35 (95% CI: 1.20-1.52), suggesting a modest but statistically significant increased risk of NAION among semaglutide users compared to non-users. The forest plot highlights the heterogeneity among the studies (I² = 85.6%). Four studies suggested a statistically significant increased risk of developing NAION, and three studies reported no significant association. Conclusion: The pooled analysis suggests a statistically significant increase in risk of developing NAION associated with semaglutide use, but the absolute increase in risk appears small, and not clinically significant, considering the low baseline population incidence of NAION (around 10 per 100,000 per year).
