08-5: EFFICACY OF INTRAMUSCULAR AUTOLOGOUS ISLET TRANSPLANTATION AFTER EXTENDED PANCREATECTOMY

Introduction: Autologous islet transplantation (IM-AIT) aims to preserve insulin secretion and reduce diabetes risk post-extended pancreatectomy. Intramuscular transplantation may mitigate risks associated with intraportal transplantation. This phase II multicentric prospective study assessed the safety and efficacy of IM-AIT in preventing diabetes. Methods: Nondiabetic patients undergoing extended pancreatectomy for non-malignant, non-genetic pancreatic diseases were included. Islets were isolated from non-pathological pancreatic tissue and injected into the brachioradialis muscle under local anesthesia on day 1 (Figure 1)The primary outcome was islet transplantation success, defined at 3 months as an increase of at least 30% in the acute insulin response to intravenous arginine (AIRa) in the transplanted arm compared to the non-transplanted arm. The secondary outcomes included islet graft function, assessed at 3 months as an increase in AIRa in the non-transplanted arm before and after vascular graft exclusion using arm-cuff inflation, and metabolic balance, evaluated at one year. Results: Between 2013 and 2022, 14 patients underwent IM-AIT following left pancreatectomy (n=11), total pancreatectomy (n=2), or pancreaticoduodenectomy (n=1) for chronic pancreatitis (n=3), intraductal papillary mucinous neoplasms (n=4), neuroendocrine tumors (n=4), solid pseudopapillary neoplasm (n=1), mucinous cystic adenoma (n=1), and trauma (n=1). A total of 101,554 ±79,775 islets with 44% ±21 purity and 92% ±4 viability were transplanted. At 3 months, IM-AIT success was achieved in 3/14 (21%) patients (Figure 2AB). 10/14 (71%) patients had a functional graft, as evidenced by a significant AIRa decrease with vascular exclusion (p=0.018) (Figure 2CD). At one year, mean HbA1c increased from 5.4 ±0.3% to 6.0 ±0.6% (p=0.008) and only 7/14 (50%) patients were independent from insulin.Conclusions: IM-AIT demonstrated islet graft survival and function in 71% of cases without major complications. This minimally invasive approach mitigates metabolic consequences of pancreatectomy but has limited success in achieving optimal function and insulin independence, highlighting the need for further optimization.
