REASONS WHY EMERGENCY HEALTH CARE IS DIFFICULT TO PROVIDE ACCORDING TO THE OPINION OF EMERGENCY HEALTH CARE PERSONNEL

Significance. Emergency health care is a mass, accessible and free type of health care in emergency situations, for conditions requiring urgent medical intervention, and the performance of emergency care service directly affects the efficiency and quality of health care delivery to population. Purpose: to explore the opinion of emergency medical specialists about challenges associated with emergency care delivery and identify possible solutions. Material and methods. A sociological survey of 904 employees of nine intermunicipal emergency care departments in the Republic of Bashkortostan was undertaken. The survey was conducted using an anonymous online questionnaire. The survey questions were aimed at exploring the opinion of employees about the reasons that complicate emergency care delivery. Results. According to 63.9 out of 100 respondents, the most significant factor that hinders timely delivery of emergency care is transport accessibility - poor road conditions, lack of street name signs and house numbers, difficult off-street access. Also, 40.6 out of 100 respondents noted poor technical conditions of vehicles due to wear and tear. Among major factors complicating the provision of emergency care at the call address, 63.3 out of 100 respondents pointed out rudeness of patients and/or their relatives, 49.6 - difficulties with medical evacuation of patients in need of hospitalization, 41.4 - aggressive behavior of the patient's relatives. Among the reasons leading to psycho-emotional overstrain of the employees, overtime work (45.9 out of 100), filling out a large number of medical documentation (46.3 out of 100), and understaffed mobile teams – only one paramedic or one doctor (23.0 out of 100) were mentioned. The need for legal protection of mobile team members (56.7 out of 100 respondents), occupational injury insurance (42.6 out of 100 respondents), and professional liability insurance was also identified. Among measures to attract personnel into the emergency care service, the respondents stated an increase in wages (65.0 out of 100 respondents) through additional payments for long-term service, additional incentive payments to young specialists (48.0 out of 100 respondents), provision of company-funded accommodation (29.0 out of 100 respondents), and creation of good workplace amenities (29.2 out of 100 respondents). Conclusion. The identified problems that make it difficult to provide emergency care are associated with the specific nature of the emergency care service and require decisions both at the intra- and interdepartmental levels. Monitoring the opinion of emergency care providers can help to timely respond to any problematic situations that may arise. Keywords: emergency health service; medical workers; sociological survey
