The acardiac twin, or TRAP (twin-reversed arterialperfusion) sequence, or acardius acranius or acardiac mass is a severe pathology of monochorionic twin pregnancies. It is characterized by the absence of development of cardiac structures, associated with a spectrum of developmental and reductional malformations in the fetus called acardiac mass. We report a case diagnosed at 23 weeks in a 37-year-old multiparous woman. The ultrasound appearance is that of an oedematous mass in the upper part in place of the thorax and the fetal head, associating fluid and echogenic zones.
