‘In life it is the little things that can make the biggest difference.’ In other words, the moments that often go unnoticed are the ones that truly matter to people with learning disabilities and those that care for them. In this book we have highlighted some of the negative moments that have led to a miserable life or even death; for example, the use of dismissive language and terminology used (being referred to as ‘having attention-seeking behaviour’, ‘having challenging behaviour’); not responding to signs of health deterioration; through institutional discrimination towards individuals with a learning disability which occurs in pockets throughout the NHS, leading to a failure to attend healthcare settings, misdiagnosis, lack of appropriate care and diagnostic overshadowing. Mostly this book highlights those positive moments and the little things that often have gone unnoticed which are having a positive impact and which truly matter. For example, a reminder that we should all be saying or writing ‘person with a learning disability’ and that we should be ensuring that we use valuing language; the use of low-arousal techniques which can create a work culture of reflection and humility; and the useful resources, top tips and think points threaded throughout this book range from little things you can do to much bigger things. We have given you insight and, in our opinion, useful advice on, for example, how to help recognise pain, understand what is happening, how to provide structure and predictability, and introducing affective reasonable adjustments to help the individual in your care. Throughout this book almost every author has also stressed the importance of effective communication. They have provided you with ideas and resources which we hope you will remember and refer to time after time, so that your little things make the biggest difference and do get noticed because your care is recognised as being exemplary, or because you receive recognition from the individual, their family, their carers, your practice assessor, your personal tutor as making a positive difference.
