Identifying people experiencing homelessness through COVID-19 hospitalization records in Los Angeles County, 2020–2022

Introduction This study aims to validate homelessness indicator variables found in COVID-19 hospitalization data from Los Angeles County (LAC).Method COVID-19 positive inpatients from LAC hospitals were identified as presumed People Experiencing Homelessness (PEH) with indicators derived from medical record address information, ICD-10 diagnoses codes and a homelessness flag required in COVID-19 mandated surveillance reporting. Analyses were conducted to understand how accurately these indicators captured the experience of homelessness compared to the results of COVID-19 contact tracing and investigation efforts. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were determined for each indicator, as well as for all indicators combined.Result There were 4,022 COVID-19 positive inpatients with homelessness indicators in LAC from March 2020 through November 2022. Of these, 67% (n = 2,711) were confirmed to be PEH. Address-indicated cases had the highest proportion of confirmed PEH (PPV = 85%), followed by ICD-10-indicated cases (PPV = 79%) and surveillance-indicated cases (PPV = 64%). NPV for each indicator exceeded 98%. Sensitivities of each indicator ranged from 31% to 39% and increased to 65%, when all indicators were combined. Specificities for all groups exceeded 99%.Conclusion While each indicator identified a large proportion of PEH, incorporation of multiple indicators increased the accuracy of homelessness detection in hospitals.
