Background: Neutrophil to lymphocyte ratio (NLR) test is shown to have prognostic value in atherosclerotic disease. There is limited data on investigating its role as a prognostic indicator of re-admission in patients with peripheral vascular disease (PVD) post angioplasty. We conduct a retrospective cohort study to investigate this. Methods: This is a retrospective cohort study conducted in Galway University Hospital from 2019–2024. Electronic hospital records were searched to identify patients. Patient data was recorded and anonymised. The neutrophil and lymphocyte ratios were calculated and analysed. Results: A total of 81 patients were identified to have received lower limb angioplasty. Of these, 21 patients required subsequent re-admission and 60 patients did not. The mean baseline NLR of patients returning post angioplasty versus non-returning was 4.99 versus 6.71 (P=0.48). The mean post-operative NLR in the return versus non-returning cohort was 5.96 versus 6.4 (P=0.42). The calculated NLR in both return and non-returning patients was comparable in both cohorts of baseline NLR and post-NLR and therefore of no prognostic value. This is in contradiction to previous studies showing a correlation of NLR with cardiovascular outcome. A possible explanation for this result is that NLR is not a specific test and can be elevated in other disease processes. This would add a significant confounder to our results. Conclusions: Our results show limited value of the NLR test post lower limb angioplasty in patients with PVD. Larger studies may improve statistical significance.
