EP6.115 Arthroscopic fixation with absorbable suture anchors for Pipkin Type I femoral head fractures—Letter V technique

Abstract Purpose: Traditional surgical treatment for femoral head fractures is effective but carries risks and major injure. Arthroscopic surgery is a safer alternative. We propose a new arthroscopic technique with absorbable anchors. Here we aim to describe a novel arthroscopic surgical technique for treating Pipkin type I femoral head fractures. Method: We explain the technique using a real case of a young adult with a femoral head fracture. The patient is positioned supine on fracture table. The basic surgical steps are similar to hip arthroscopy procedures. Initially, intra-articular loose bodies and acetabular labral injuries are addressed, followed by traction release. The anteriorly displaced femoral head fragment is exposed in a flexed hip external rotation position and reduced. Subsequently, three absorbable anchors are used for full arthroscopic fixation: two in the central area of the displaced fracture fragments provide initial stability by passing through the fracture fragments, and one in the femoral head. The third anchor is tied with the other two anchors using a double-pulley technique to form a suture bridge further securing the fracture in a “letter V” shape, providing convenient and stable fixation for appropriate femoral head fractures. Result: The surgery was successfully completed, the fracture was well reduced, postoperative joint mobility was good, and after rehabilitation, the patient’s postoperative function significantly improved. Postoperative MRI images showed good healing of the fracture. Conclusion: Treating femoral head fractures with absorbable suture anchors and a suture bridge (letter V) technique arthroscopically offers the advantages of simplicity, convenience, and safety.
