Impact of sleep-based mobile vital sign monitoring in therapy and supervision of chronic obstructive pulmonary disease, cystic fibrosis and other lung diseases

Introduction COPD and cystic fibrosis CF are characterized by progressive chronic respiratory failure ultimately leading to death. CF is an autosomal recessive condition, which affects multiple organ systems, while COPD is associated by persistent airflow limitation caused by chronic inflammatory disease in the airways and the lung. However, patients suffering from those diseases are more vulnerable to have bad disease progression often associated with exacerbations. Because of this, an early recognition of respiratory infections or deterioration of the lung function are important to start medical treatment as early as possible, which may result in better life quality and lower mortality. Here, patients with COPD or CF are monitored during hospitalization using a minimal-contact biomotion device. Methods Changes in vital signs with focus on respiration and sleep characteristics are analysed. Individual natural variations are taken into account. In particular, it is considered that the algorithms and results are easy to understand in order to facilitate application into clinical practice. Results We were able to show a high correlation between device and polysomnography for respiratory rate (r=0.99) as well as heart rate (r=0.99). Additionally, we were able to detect breathing abnormalities are detected with a sensitivity of 82.42% and central apnea are recognized with a sensitivity of 83.33% based the cardiorespiratory polysomnography recording and minimal-contact device signal recording of a single patient. During this research at the Ruhrlandklinik (University Hospital Essen, Germany), we aim to identify further patterns in vital sign for monitoring patient disease progressions and will be prospectively used for predicting those progressions. Conclusion Summarizing, the current algorithms show promising results for monitoring breathing and its disorders during sleep. In the future, the combination of mobile vital sign monitoring and signal analyzing has a high potential for patient disease progressions.
