ASSOCIATION OF POLYCYSTIC OVARY DISEASE AND INSULIN RESISTANCE IN OBESE AND NON-OBESE WOMEN

Background: Polycystic ovary syndrome (PCOS) is a common endocrine disorder affecting 5–20% of reproductive-aged women worldwide, contributing to metabolic and reproductive dysfunction. Insulin resistance (IR) is a key feature of PCOS, exacerbating hyperandrogenism and increasing the risk of type 2 diabetes and cardiovascular disease. While obesity is a known contributor to IR, non-obese women with PCOS also exhibit metabolic disturbances, highlighting the need for further investigation into the relationship between PCOS and IR across different BMI categories. Objective: To determine the association between PCOS and insulin resistance in obese and non-obese women. Methods: A cross-sectional study was conducted from June to December 2023, enrolling 220 women diagnosed with PCOS. Participants were categorized into obese (BMI ≥30 kg/m², n=110) and non-obese (BMI <30 kg/m², n=110) groups. Diagnosis was based on the Rotterdam criteria. Postmenopausal women and those with other endocrine disorders were excluded. Anthropometric measurements, fasting glucose, fasting insulin, and lipid profiles were assessed. Insulin resistance was determined using the homeostasis model assessment of insulin resistance (HOMA-IR), with a cutoff of >2.5. Statistical analysis was performed using SPSS v29, with comparisons between groups conducted using independent t-tests and logistic regression analysis. Results: Obese women had a significantly higher BMI (32.8 ± 2.9 vs. 24.1 ± 2.3 kg/m², p<0.001) and waist circumference (91.3 ± 8.2 vs. 76.2 ± 6.5 cm, p<0.001). Fasting glucose (5.6 ± 0.8 vs. 5.1 ± 0.6 mmol/L, p=0.02), fasting insulin (18.5 ± 5.2 vs. 12.2 ± 3.8 mU/L, p<0.001), and HOMA-IR scores (4.6 ± 1.1 vs. 2.8 ± 0.7, p<0.001) were significantly higher in the obese group. Insulin resistance was observed in 78% of obese and 43% of non-obese women. Obesity was a strong predictor of IR (OR=3.4, 95% CI: 2.1–5.5, p<0.001). Conclusion: Insulin resistance is prevalent in both obese and non-obese women with PCOS, with obesity amplifying its severity. These findings highlight the need for individualized management strategies addressing insulin resistance independent of BMI to improve metabolic and reproductive outcomes. Early interventions may reduce long-term cardiovascular risks in affected women.
