How Do I Manage My Patient in the Hospital with Intractable Pain?

Abstract This chapter focuses on patients with sickle cell disease (SCD) who have episodes of pain that lead to long hospitalizations with very slow improvement, despite aggressive use of conventional measures. There is no consensus definition of a refractory vaso-occlusive crisis (VOC). Yet many treatment guidelines and expert opinions offer guidance on treatment of refractory VOCs. This chapter synthesizes the literature and various recommendations with the author’s experience to provide a strategy for assessing and managing a patient with severe SCD pain in the hospital that is not responding to well-executed conventional methods.
