Evaluation of a Novel Medication Follow-Up Support Sheet Administered to Patients with Diabetes by Community Pharmacists

Since September 2020, Japanese pharmacists have been required to provide continuous medication follow-up to patients in order to improve their interpersonal services. However, the approach to conducting this follow-up currently depends on individual pharmacies or pharmacists. In this study, we developed a novel medication follow-up support sheet (referred to as “support sheet”), based on the findings of previous studies, to support pharmacists in Japan in providing effective medical therapies to patients with diabetes. After three months of medication follow-up using the support sheet, pharmacists responded to our questionnaire regarding its availability. The responses from 22 of the 29 responding pharmacists were considered valid (76％ response rate). A customer satisfaction analysis revealed that the items requiring improvement were those related to checking the patients’ understanding of their medications, the pharmacists’ understanding of the medication follow-up procedure, and the visual ease of making the follow-up record. In contrast, items related to medication status and confirmation of the occurrence of adverse events were considered to be crucial components. The free comments provided by the pharmacists in the questionnaire demonstrated a generally positive response towards the three items “medication status,” “understanding of medication,” and “occurrence of adverse events” that were set as the contents of the medication follow-up. These results suggest that the novel developed support sheet serves as medication follow-up for patients with diabetes and contributes to the provision of safe and optimal medical care to patients.
