Characteristics of severe asthma patients treated with biotherapy: the French Palomb Cohort.

Introduction: Biotherapies are innovative treatments that help to reduce corticoids intake but also exacerbations, dyspnea, quality of life and corticoids side effects among severe asthma patients. This work aims to describe characteristics of severe asthma patients with biotherapy treatments in France. Methods: Multicentered prospective cohort of patients included and followed-up since 2019. Recruitment in 2 French regions by pulmonologists. Inclusion criteria: ⩾ 18 years-old ; > 6 months follow-up before inclusion for severe asthma (definition: GINA 2019) ; ICS-LABA combination and anticholinergic, associated with OCS, anti-IgE based treatment, anti-IL5, anti-RIL5, anti-IL4, anti-IL13, thermoplasty. Results: In February 2024, 267 patients were included. 63.5 % were women and mean age was 54.1 years old (±17.4). A total 146 (54.7%) already had a biotherapy treatment at baseline and 35 (13.1%) received it when included. Among those who were prescribed biotherapy at baseline (n=35), the aim of it was to decrease the risk of exacerbation (77.1%), improve asthma control (74.3%), quality of life (57.1%), FEV1 (42.9%) and wean OCS (25.7%). This same group of patients had more wheezing, expectoration, nocturnal respiratory signs and a greater number of exacerbations in the past 12 months (≥ 2) compared to those already treated with biotherapy and those without it at baseline (p<0.05). Patients already with biotherapy were more receiving omalizumab and patients receiving it at baseline were more treated with dupilumab.
