Introduction: Optimising functional and HRQoL outcomes are important goals following LTx. Aim: To examine the longer-term effects of a digital physical activity (DPA) intervention on health outcomes following LTx. Methods: Following hospital discharge, LTx recipients were randomised to 3 months of a DPA intervention or usual care (UC). DPA consisted of a home exercise booklet and PA monitoring using a pedometer and smartphone app, which sent data to a central platform that provided feedback, activity goals, education and contact with the researcher. Outcomes assessed were accelerometer physical activity (PA) outcomes, HRQoL (SF-36), mortality and intervention adherence. Results: 24 LTx recipients were recruited (ILD=14; COPD=6; CF=2; PAH=2) and randomised (DPA=13, UC=11). At 12 months, 4 patients were lost to follow up (DPA=2, UC=2) and 2 patients were deceased in the UC group. Adherence to uploading PA data over the 3-month intervention was 89±16%. From baseline to 12 months, DPA showed significantly greater improvements in PA outcomes, compared to UC (Fig 1). Similarly, DPA exhibited a clinically greater improvement in SF-36 physical scores compared to UC, but there was no difference in SF-36 mental scores (Fig 1). Conclusion: A DPA intervention following hospital discharge led to improved health outcomes at 12 months and could be a promising intervention to promote recovery.
