Aims & Objectives: Background and objective: Stroke is a time-dependent pathology in which early diagnosis and treatment are essential to reduce morbidity and mortality. In children the incidence of stroke is low and the diagnosis is frequently delayed because other pathologies present similar symptoms. The objective of the study was to analyze the characteristics and evolution of stroke in children. Methods: A prospective unicenter registry was carried out from 2019 to 2023 of pediatric patients in whom the stroke code was activated, analyzing clinical characteristics, diagnostic methods, treatment and evolution. Results: 124 stroke codes were activated and the diagnosis of stroke was confirmed in 34%. The median age of children with stroke was 7.8 years. 69% were boys. 23.9% had heart disease. In 30.6% of strokes were initially in-hospital (12.7% in the PICU). 63% of strokes were ischemic, 30.4% hemorrhagic, 2.2% venous sinus thrombosis and 4.3% mixed. The most frequent symptoms were motor symptoms (52.2%), headache (34.8%), alterations in consciousness (32.6%) and seizures (21.6%) with no differences in symptoms among stroke patients and those who did not present it. The diagnostic methods were CT or angioCT (39%), MRI (17.5% or both (43.5%). Thrombectomy was performed in 2 patients (4.3%) and fibrinolysis in another 2 (4.3%). 19.6% of patients required surgery. Mortality was 19.6%, leaving 45.6% of patients with neurological sequelae. Conclusions: stroke in children is a serious pathology with high mortality and sequelae in survivors. Although 60% of strokes are ischemic, the percentage of patients treated with fibrinolysis and thrombectomy is low. Keywords: fibrinolysis, code stroke, stroke, Thrombectomy
