Aims & Objectives: Emergency airway management is a resuscitation procedure in the ED. NEAR4KIDS study, 1715 intubations, 62% had first atempt success rate, 20% of intubations were associated with adverse events.JEAN4 was carried out in 5 pediatric emergency departments, 231 patients, obtaining a first attempt succes rate of 72%, the lowest percentage in children under 2 years of age. Primary Aim: Description of the intubation process and first atempt success rate and presence of associated adverse events. Secondary: Describe the presence of a difficult airway. Methods: Prospective, cross-sectional, study. July 2017 to March 2023. Inclusion criteria: patients under 18 years of age, admitted to the ER with an unplanned orotracheal intubation. All information obtained from the collection sheet. Results: 773 intubations. Difficult airway: in 21.5% of the cases. 76.5% intubated by the pediatric ED fellow, with a success rate of 53% on the first attempt. 22% of adverse events were found. Less probability of complications for each less hour of work of the intubating person. For each intubation attempt, the risk of adverse events increases (OR.17 95% CI (1.1-2.3)), with a difficult airway there is a 1.7 times greater chance of presenting adverse events (95% CI 1.3-5.3). For a second person intubation the risk of adverse events is 3.2 times (95% CI 2.7-7). If it is a difficult airway the risk increases 5.1 times (95% CI 3.1-10). Conclusions: First attempt succes rate was 53%, with 22% adverse events, which occurs more frequently if it is a difficult airway and if there are 2 or more attempts at intubation.
