The effects of fracture or disease on biochemical markers of bone remodelling

Biochemical markers of bone remodelling have comprised an important adjunct to clinical examination and radiographic studies in the diagnosis of metabolic bone diseases for at least 50 years. They have also been the mainstay of monitoring progression or regression of metabolic bone diseases. The newer markers of remodelling currently employed in patients with osteoporosis, where bone remodelling abnormalities are less pronounced than in other metabolic bone diseases, have improved sensitivity over the older markers but offer no apparent improvement in diagnostic specificity. Skeletal fractures heal by local remodelling, and the extent to which this remodelling is reflected in biochemical markers is most probably related to the extent of the fracture. Specific questions that cannot yet be fully answered from the available data include the following: (1) Are there levels of markers that indicate the presence of a secondary cause of bone loss or metabolic disease other than osteoporosis? (2) Are there patterns of abnormality in markers that offer differential diagnostic information? (3) To what extent and for how long do common osteoporotic fractures influence levels of markers? (4) Is there a minimum amount of immobilization that is likely to affect levels of markers?
