An in-vitro comparison of different caries-detection methods in primary-molars

Early and accurate diagnosis of caries, particularly in pediatric dental patients, leads to precise treatment planning. This in vitro study aimed to evaluate and compare the performance of different detection methods for the detection of occlusal-caries in the primary-molars. 84 occlusal-surfaces of freshly extracted human primary-molars evaluated by two different examiners by using; International-Caries-Detection and Assessment-System (ICDAS), light-emitting-diode-device (Midwest Caries I.D), Fluorescence-based devices (DIAGNOdent pen) and Quantitative-Light-induced-Fluorescence (QLF). All teeth assessed twice with a two weeks interval by both examiners using all 4 methods. Histological evaluation was used as the gold standard. Sensitivity, specificity, and accuracy were calculated. Correlation with histological analysis was evaluated using Spearman's rank correlation coefficients and ROC analysis. Weighted Kappa and intraclass-correlation (ICC) coefficients were calculated to assess intra-examiner reproducibility. The sensitivity values obtained by the methods were as follows: QLF (0.95), ICDAS (0.78), DIAGNOdent pen (0.65), Midwest Caries I.D (0.65). Inter-and intraexaminer intraclass-correlation-values varied from 0.772 to 0.963 and unweighted kappa-values ranged from 0.762 to 0.850. Given the significance of early detection of caries in primary-teeth, the use of Midwest Caries I.D, QLF, and DIAGNOdent devices, in conjunction with conventional-examination methods, can enhance the identification rate of occlusal-caries during routine clinic visits.
