An 18-year-old female with a body mass index (BMI) of 35.1 kg/m 2 presented to the emergency room (ER) with nausea and intractable vomiting.She reported 3 to 4 episodes of nonbloody, nonbilious emesis, and epigastric pain that began 4 hours prior.During initial assessment, the patient disclosed that she had been fasting for Ramadan for 2 to 3 days consecutively, deviating from the traditional practice of breaking the fast each evening.She also shared a history of restrictive eating, which had resulted in a 50-pound weight loss 3 years ago.She stated that this weight loss was recommended by her pediatrician.She denied understanding the seriousness of the condition and stated she would likely try to fast again after hospitalization.Upon inquiring about use of medications for weight loss, she admitted to taking her mother's semaglutide injection (Wegovy ) 4 hours before arriving to the ER.She had previously used her mother's Wegovy months earlier without adverse effects.Vital signs and physical exam in the ER were unremarkable: Blood pressure (BP) was 118/79 mm Hg, pulse was 77 bpm, respiratory rate was 16 breaths/min, and temperature was 97.9°F.
