Dermatomyositis associated with anti-programmed cell death (PD)-1 therapy

Programmed cell death 1 (PD-1) blockade therapies emerged as effective treatments for a wide variety of malignancies.However, these therapies have been associated with multiple immune-related adverse effects, including cutaneous eruption.Here, we present two cases of patients who developed clinical findings consistent with dermatomyositis after receiving PD-1 inhibitor therapy for malignant pleural mesothelioma and pulmonary squamous cell carcinoma.The first patient continued immunotherapy for malignant pleural mesothelioma and subsequently developed dermatomyositis with a positive anti-Ro 52 antibody .The second patient, despite discontinuing anti-PD-1 therapy, presented with dermatomyositis posotive for both anti-Ro 52 and anti-TIF1γantibodies.In both cases, the dermatomyositis was resolvedfollowing treatment with high doses of glucocorticoids combined with gamma immunoglobulin.These findings add dermatomyositis to the growing list of autoimmune complications associated with PD-1 blockade therapy.These cases raise serveal concerns, including the viability of continuing anti-PD-1 therapy after the development of dermatomyositis and the significance of early symptoms in patients with PD-1 inhibito-associated dermatomyositis.
