Guidelines on the management of anticoagulant in emergency setting

ObjectiveFor the training of junior doctors (DJ) in medical regulation, we use a card game to determine by lot the scenarios for the simulation sessions. The aim of this study was to check that the situations proposed were in line with the reality of calls handled at SAMU 93.MethodsThe card set evaluated consisted of 53 cards in four sets corresponding to (1) the caller’s mode of communication (7 cards, referring to Snow White’s Seven Dwarfs), (2) the relationship between caller and patient (14 cards), (3) the patient’s age (11 cards) and (4) the reason for call (21 cards). The investigators were recently trained DJs (N=7) and a supervisor. In successive calls, they recorded the patient’s age and gender, the caller’s relationship with the patient, his or her mode of communication and the reason for referral (multiple responses possible). The objective was to include 250 calls.Results328 consecutive calls included by six investigators: 150 (46%) men and 178 (54%) women with a median age of 41 (23-68) years. Callers in descending order: themselves (28%), a carer (16%) and a teacher (11%). Five cards were added. Communication mode in descending order: “normal” (45%), “simpleton” (16%) and “shy” (12%). Reasons for referral: 70, including abdominal pain (11%), chest pain (10%) and fall from height (10%). Trauma accounted for 75 (22%) calls. Twelve cards were added.ConclusionThis study has enabled to validate a realistic card game in line with the activity of SAMU 93 to determine the scenarios for simulation sessions, with 51,205 possible combinations. This game could be generalized and adapted to local specificities.
