Clinical experience of segmentectomy of right middle lobe of lung

Background: In recent years, segmentectomy has shown promise as a limited alternative to lobectomy for curative resection of early-stage lung cancer. However, segmentectomy of the right middle lobe is rarely performed because its benefits are considered marginal. Here, we report our experience with this procedure for lung resection.Patients and Methods: From 2012 to 2023, there were five cases of lateral segment resection and one case of medial segment resection at our institution; one of the lateral segment resection cases also included superior segment resection.Results: The mean age was 68 ± 6.2. The pathological diagnosis was primary lung cancer for five and a benign tumor for one. The tumor size was 20 ± 8.5 mm. All surgeries were performed with a video-assisted thoracoscopic procedure. There were no severe postoperative complications. Postoperative CT confirmed that the remaining segment of the middle lobe was sufficiently inflated in each case.Conclusion: Segmentectomy of the right middle lobe is technically possible to perform for specific patients with a small lesion, for whom it is difficult to conduct wedge resection.
