The Physiology of Bone Homeostasis to Aid Experimental Therapeutics in Disease Conditions

Bone homeostasis is regulated by osteoblasts (bone-forming cells) and osteoclasts (bone-resorbing cells) through two processes: modeling and remodeling. Bone modeling forms mature bone during growth and development; during bone remodeling, bone resorption is followed by bone formation as osteoblasts and osteoclasts act sequentially to deposit new bone at the exact spatial location. An imbalance in this process leads to various bone metabolic disorders. In this chapter, we first discuss different bone cell types and cellular and molecular events during skeletal homeostasis, as well as other pathophysiological situations that give rise to different bone diseases. Following that, we discuss cell culture systems and assays for discovering therapeutic entities and preclinical models for osteoporosis (primary, secondary, and senile) and fracture healing, their relative advantages over one another, and their similarities in light of human disease conditions. Furthermore, we discuss the therapeutics available for osteoporosis and their limitations. Finally, we critically assess the antiosteoporosis potential of certain botanicals/herbal medicines, including discussing their molecular effects.
