One of the main advantages of screw retention is retrievability in the event of screw loosening, screw fracture, or other prosthetic and biological complications. Without a screw-access hole, cement-retained implant restorations can be difficult if not impossible to remove and often require irreversible damage to the restoration and potential damage to the implant. One method of improving retrievability for cement-retained restorations is the inclusion of additional screws into the framework design, such as the lingual set screw. This chapter discuses the strategies to minimize cement excess expressed into gingival sulcus. The presence of residual cement in the sulcus is a risk factor for bleeding, abscess formation, and inflammation in the peri-implant tissues with the potential for attachment loss and implant failure. In summary, when considering implant prosthesis retention, carefully designed abutments, which incorporate two vent holes can be successfully utilized with cement retention.
