Pupil expansion devices help to deal with the issue of intraoperative small pupils effectively. In complicated and challenging cases, pupil expansion devices are often employed to achieve a clear and unobstructed view of intraocular structures as adequate pupil dilation provides the surgeon with better access to the intraocular structures and aids in adequate and proper management of the complication. In cases of posterior capsule rupture (PCR), iris hooks are a safe alternative as compared to rings because there is no fear of iris hooks getting dropped into the vitreous cavity. According to Drs. Bryan Lee and David Chang, as explained in Chapter 8 , “The intensity of the red reflex is significantly reduced with each millimeter less of pupil diameter. For instance, the constriction of a pupil from 7 mm dilation to 5 mm would reduce the area for the red reflex by about 50%, which can make it difficult to perform the capsulorrhexis because of poor visibility.”
