Tuberculid caused by atypical mycobacteria in an immunocompetent patient: a rare case report

Tuberculid is defined as cutaneous hypersensitivity eruptions to M. tuberculosis that occur in patients with moderate or high levels of immunity against the microorganism. We report a rare case of long-lasting tuberculid of the nasal bridge caused by atypical mycobacteria. A 35-year-old male patient was diagnosed with pulmonary tuberculosis at the age of 19 years. He received anti-tuberculosis therapy (ATT) for 9 months, and 14 years ago (1 year after completing ATT), a nodular lesion appeared over his right cheek, which gradually spread to the nasal bridge and resulted in facial disfigurement. A biopsy was performed, and histopathology revealed features consistent with lupus vulgaris. PCR for atypical mycobacteria was positive. The patient was treated with clarithromycin, rifampin, and ethambutol. Key words: tuberculid, atypical mycobacteria, immunocompetent, lupus vulgaris
