Epidemiological studies have shown that the prevalence of Von Willebrand disease (VWD) may be as high as 1%. This figure has been obtained by using conservative clinical and laboratory criteria. However, many of the subjects identified by this means will experience only minor or trivial bleeds during their lifetime, and most of them will probably never be referred for medical assistance. The prevalence of cases referred to specialized centers for the treatment of bleeding can be estimated to be around one case in every 10,000 subjects, a figure similar to the cumulative prevalence of hemophilia A and B. Severe VWD is a rare disorder with a prevalence of around 1 case/1,000,000, depending on the ethnic background. Although about 1% of the normal population could satisfy quite conservative epidemiologic criteria sufficient to diagnose VWD in ad hoc cross-sectional investigations, a diagnosis of VWD in clinical practice should be pursued through standardized clinical tools (e.g. bleeding score), when a clear benefit for the patient and his/her family will derive in terms of prophylaxis and treatment.
