Computed tomography and Doppler ultrasound in choosing surgical strategy for tandem stenosis of common and internal carotid arteries in a patient with coronary artery disease

Objective. To demonstrate a patient with concomitant coronary and tandem carotid lesions regarding the choice of visualization technique. Material and methods. A 60-year-old man admitted to cardiovascular department for coronary artery bypass grafting. Preoperative examination revealed significant atherosclerotic lesions of both internal carotid arteries and tandem stenosis of one common carotid artery. Results. We demonstrate successful surgical treatment of a patient with CAD and complex carotid lesions. Abilities and benefits of ultrasonography and CT-angiography in measurement of carotid stenosis are analyzed. Conclusion. This clinical situation is rather rare, and there is no consensus regarding surgical strategy. Ultrasonography and CT-angiography were valuable to clarify localization and nature of carotid artery lesions. We were able to determine surgical treatment of a «difficult» patient with multifocal atherosclerosis and evaluate the effectiveness of intervention 3 months later.
