Three main components of initial enteral support for neurological patients in critical illness

Protein-energy deficiency in cerebral stroke is associated with a decrease in the rehabilitation potential of patients, an increase in the number of complications and length of stay in the intensive care unit (ICU), and the risk of higher mortality. The article discusses the basic concepts of prescribing enteral nutrition for cerebral stroke and traumatic brain injury (TBI): early start of nutritional support with priority use of enteral formulas (timeliness); preventing overfeeding of a patient in the acute phase of a critical condition using an algorithm for gradual delivery of protein and energy (stepping); the use of specialized enteral formulas with a high protein and a moderate calorie content (specialization) in order to achieve protein/calorie intake in the acute phase and reduce mortality rates. This approach makes it possible to initiate nutritional support for cerebral stroke and traumatic brain injury (TBI) in compliance with current clinical recommendations.
