Extremity vascular injury caused by trauma remains a significant cause of mortality and morbidity in the injured patient. With technological advances, vascular surgery has changed dramatically over the past decade. Trauma surgeons are faced with these injuries daily and need to have the expertise to take care of these patients acutely. There is a growing body of evidence on which to base both diagnostic and therapeutic decisions in the acutely injured trauma patient with extremity vascular injury. This chapter reviews the recent literature related to the diagnosis and management of acute vascular extremity trauma, divided into relevant clinical questions, with recommendations based on the most recent evidence. Physical exam can adequately identify findings concerning extremity vascular injury. The use of tourniquets for the management of prehospital control of peripheral vascular trauma has the potential to save lives with minimal morbidity.
