Structured exercise during cancer treatment can improve fitness, health-related quality of life (HQRL), perioperative outcomes, and reduce mortality and recurrence1–4. However, recruitment rates for exercise trials in patients with cancer are low and drop-out rates are high5. Neoadjuvant chemotherapy (NAC) and oesophagogastrectomy for oesophageal cancer has a negative effect on HRQL and cardiovascular fitness6,7. Studies2,8 of prehabilitation before oesophagectomy have shown promising results in mitigating these effects. However, in contrast to findings in patients undergoing surgery for colorectal cancer9, they have not demonstrated a survival advantage. This study examined the long-term outcomes of PRE-EMPT (Prehabilitation of patients with oEsophageal Malignancy undergoing Peri-operative Treatment trial), a non-randomized prospective trial that compared a group of patients undergoing a prehabilitative exercise intervention with a control group receiving standard treatment. Also examined were the outcomes of the prospectively recorded patients who dropped out of the trial or declined to participate. Pre-EMPT was a non-randomized, interventional study that assessed the effects of a structured exercise programme in patients undergoing NAC and surgery for adenocarcinoma of the lower oesophagus and gastro-oesophageal junction4. The study received ethical approval (REC 16/SC/0438) and was registered at ClinicalTrials.gov (NCT03626610). All patients undergoing oesophagectomy in our institution are entered into an ethically approved database (IRAS reference: 12-NW-0511).
