Systemic absorption of topical IMQ can occur and may account for PSO occurring at distant sites from treated areas, such as in our patient.Most of the previous reports of imiquimod-induced PSO had a personal or familial history of PSO and were well managed using topical corticosteroids or calcipotriol.To the best of our knowledge, there are few reports of extensive imiquimod-induced PSO reported, and infliximab was used in one of them with erythrodermic arrangement.2,5 It is important to consider the possibility of imiquimod as a culprit for induced extensive PSO.A thorough medical history and histopathological examination are essential for accurate diagnosis and management.
