Using Ondansetron to Reduce Vomiting During Bowel Preparation for Colorectal Surgery: A Preliminary Analysis of A Prospective Study

Introduction: Mechanical bowel preparation with oral antibiotics is recommended before colorectal resection, however, many patients experience vomiting. This is a preliminary analysis of an ongoing study aimed to determine if ondansetron could prevent vomiting during bowel preparation for colorectal operation. Methods: Patients undergoing colorectal resection at our institution routinely undergo mechanical bowel preparation with oral antibiotics beginning the afternoon before operation, consisting of bisacodyl, polyethylene glycol, neomycin, and metronidazole. We recently began adding a prescription for ondansetron 4mg taken at 1pm and 7pm during the bowel preparation. Patients document medication use and symptoms, including the primary outcome of vomiting, in a bowel preparation journal. For this study, we compared the incidence of side effects in patients given ondansetron compared to historical controls not given ondansetron using chi-squared tests. Results: To date, we have enrolled 49 patients (20 ondansetron and 29 control), of whom 61% were women. Vomiting during bowel preparation was reported by 25% of ondansetron patients and 37% of controls, p=0.34. Nausea was reported by 70% of ondansetron patients and 58% of controls, p=0.42. Dizziness was reported by 35% of ondansetron patients and 6% of controls p=.0125. Conclusion: This study is ongoing, however this preliminary analysis does not indicate a statistically significant decrease in vomiting among patients given ondansetron during bowel preparation before colorectal resection. the data suggest there may be tradeoffs with ondansetron such as dizziness, that need to be considered when developing optimal bowel preparation strategies.
