Assessment of the long-term safety and efficacy of dupilumab in children with asthma: LIBERTY ASTHMA EXCURSION

Introduction Dupilumab (DPL), a fully human mAb, blocks the shared receptor component for IL 4/13. The efficacy and safety of add-on DPL versus placebo (PBO) in children with asthma have been demonstrated up to 52 weeks (wks) in the VOYAGE study. Aim The EXCURSION open-label extension study (NCT03560466) assessed DPL long-term safety and efficacy in children with uncontrolled, moderate-to-severe asthma who completed VOYAGE (aged 6–11 years at enrolment in the parent study). Methods 365 patients (pts) from the DPL or PBO group of VOYAGE enrolled in EXCURSION, and received add-on DPL 100/200 mg (body-weight based) every 2 wks for 52 wks. A subgroup of pts received 300 mg every 4 wks. Treatment-emergent adverse events (TEAE), annualized rate of severe asthma exacerbations (AER), and change from parent study baseline (PSBL) in pre-bronchodilator (pre-BD) percent predicted (pp) FEV 1 were assessed. Results Safety during EXCURSION was consistent with the parent study. In pts with type 2 asthma (blood eosinophils≥150 cells/µL and/or FeNO≥20 ppb at PSBL), the low unadjusted AER and improvements in pre-BD ppFEV 1 observed with DPL in the parent study were sustained throughout the 52 wks of EXCURSION. Pts who switched from PBO during VOYAGE to DPL also showed low AER during EXCURSION, and improvements in ppFEV 1 as early as EXCURSION Wk 2, which were sustained to Wk 52 (Table). Conclusion Long-term (2 years) use of DPL was well tolerated. The efficacy of DPL observed in the parent study sustained over an additional 52 wks of DPL treatment in pts with type 2 asthma.
