Background: LAM is a rare progressive disease that leads to respiratory failure. The study aimed to describe the diagnostic accuracy of MMP-2 and compared it to VEGF-D. Methods: LAM patients with or without lung transplantation, patients with other cystic lung diseases (OCD), and healthy females were included. A single blood sample was taken. Serum MMP-2 and VEGF-D were measured using Luminex Human Magnetic Assay. ROC curve was performed to establish the optimal sensitivity and specificity. A binary logistic regression including biomarkers concentration as covariates was used to predict LAM diagnosis. Results: 97 subjects were recruited: 59 (61%) LAM patients, 18 (19%) OCD and 20 (21%) healthy women. Serum MMP-2 was higher in the LAM group with a median of 578230 pg/mL compared to OCLD and healthy controls with a median of 359980 and 427476 pg/mL, respectively (p< 0.0001). The AUC of MMP-2 was 0.785 and the combination of MMP-2 and VEGF-D increase the diagnostic accuracy with an AUC of 0.884, see Figure 1. Conclusion: Our study demonstrates that MMP-2 has a potential utility as a diagnostic biomarker for LAM which increases the accuracy of the diagnosis if it is combined with VEGF-D.
