What is the clinical relevance of reference values in the 6MWT?

The 6 minute walking test (6MWT) is widely used to evaluate functional performance, evolution and prognosis of cardiorespiratory diseases. As of today, there are several reference values which show significant variability among them. The aim of this study is to evaluate the clinical significance of distinct reference values in the 6MWT. A retrospective study with was carried out with 170 patients (53 women), from 27-93 years of age and chronic respiratory disease with a clinical prescription for a 6MWT in our hospital. The test was performed following ATS protocol. The predicted distance values were obtained applying Enright, Trooster, Osses, Halliday, Gibbons, Gimeno-Santos and Casanova formulas considering pathological if <80% of the theoretical value. The concordance between formulas was tested with Cohen´s Kappa coefficient, considering >50% significant. The population categorized as pathological varied as follows: The significant concordance was: The variability present among different reference value formulas shows poor clinical significance. The inconsistency of different reference values makes it difficult to extrapolate data between different populations.
