The history of metabolic and bariatric surgery is full of serendipity and misconceptions. Initially, MBS surgeons recognized that gastrectomy for benign and malignant disease leads to weight loss and improvement of type II Diabetes. This led surgeons to believe that MBS is restrictive only to later recognize that distal delivery of nutrients is the mechanism by which MBS works. Next, MBS recognized that certain procedures lead to protein calorie malnutrition and labelled them malabsorptive only to discover later that the size of the stomach and the total alimentary limb length [TALL] is the driving factor because the length of small intestine is variable.
