Abstract This chapter considers patients with both heart failure and valvular heart disease (VHD). As the population ages, the clinical importance and burden of VHD increases and is highly relevant to everyday cardiac care. Where symptomatology is subjective and confounded by co-morbidities, the clinician is faced with the challenge of evaluating a conundrum of complex physiological parameters while also balancing the potential risks and benefits of intervention by means of surgical valve replacement or percutaneous intervention. The emergence of transcatheter aortic valve implantation and percutaneous edge-to-edge repair for mainstream treatment of aortic stenosis and mitral regurgitation, respectively, has paved the way for a variety of innovative percutaneous options for the potential treatment of VHD. Meanwhile, there are currently no medical or pharmacological therapies to halt progression of VHD or reverse valve destruction. The chapter then considers chronic VHD and discusses cardiac physiology, clinical features, quantification using echocardiography, and clinical management for individual valve lesions.
