Bronchoalveolar Lavage Lateral-Flow Device Test for Invasive Pulmonary Aspergillosis in Solid Organ Transplant Patients: A Semi-Prospective Multicenter Study.

Invasive pulmonary aspergillosis (IPA) is an important cause of morbidity and mortality in patients after solid-organ transplantation (SOT). Due to the crude mortality (80-90%) in absence of adequate treatment, timely diagnosis and intervention with antifungal drugs are key factors in the successful treatment. Diagnosis of IPA, remains difficult as clinical signs and symptoms as well as radiological findings are unspecific and lack sensitivity. Therefore, antigen testing has become one of the cornerstones of IPA diagnostics, the tests employed suffer from varying turnaround times as well as issues of sensitivity and specificity. These limitations may be overcome by the Lateral-Flow Device (LFD) test, a rapid (15 min) single sample point-of-care test. This semi-prospective multicenter study evaluated the LFD test for IPA diagnosis by using bronchoalveolar lavage (BAL) samples from patients after SOT. 47 BAL samples from SOT patients were included (11 probable/proven IPA, 11 possible IPA, 25 no IPA) at three Austrian Medical Universities (Innsbruck, Vienna and Graz). Sensitivity and specificity, positive- and negative predictive value as well as diagnostic odds ratio of BAL LFD tests for probable IPA were 91%, 83%, 63%, 97%, 50 (95%CI 5.4-467), respectively. The LFD test of BAL specimens is performed easily and provides accurate and rapidly available results. Therefore, this new point-of-care test may be a very promising diagnostic approach for detecting IPA using BAL specimens from SOT patients.
