DOUBLE VERSUS SINGLE RENAL ALLOGRAFTS FROM AGED DONORS

22 From December 1996 to May 1998, a prospective, clinical trial was designed to extend to an unlimited age the admission of cadaveric kidney donors with normal serum creatinine. We perform simultaneous double kidney transplant (SDKT) in a single recipient under two conditions: 1) donors very old (older than 75 yr) or 2) donors between 60 and 75 yr with a glomerulosclerosis (GE) rate in the pretransplant renal biopsy higher than 15%. From Dec. 96 to May 98 (eighteen month study period), 181 patients recived a renal transplant (RT): 120 single RT from donors younger than 60 yr (Group I), 40 single RT from D>60 (Group II) and 21 SDKT from D>60 (Group III). Mean time follow up was 13±6 months (range 3-21). The inmunosuppression protocol included prednisone, micophenolate mofetil and cyclosporine microemulsion or FK506 (20 patients from Group I and 5 from Group II) or rapamycin (3 patients from Group I). In the following table we show the main results:TableIn conclusion the performance of SDKT following our protocol (donors >75 yr or donors between 60-75 yr with a GE rate >15%) shows very good results at short time. Kidneys from donors 60-75 yr with GE rate < 15% can be successfully used as single RT. It is remarkable the low incidence of primary nonfunction and acute rejection in the Groups II and III and the very low incidence of delayed graft function in the Group III.
