Background: Desynchronisation of heart electrical activation and contraction, induced by conventional right ventricular pacing, is a possible reason for heart failure development without any other underlying disease. Objective: To evaluate left atrial function during conventional right ventricular pacing by using speckle tracking imaging (STI). DesignandMethods: atotal of 47 individuals (meanage 69.77+10.50) were included in this prospective reseasch. Allwere addmited tocardiology departmentdueto conventional indications for dual chamber pacemaker implantation : sick sinus node syndrome or second to third degree atrioventricular block. Echocardiography wasperformed andevaluated by using conventional and STI techniques at baseline, on follow-up after 3 month and one year. Results:Measurement of left atrial (LA) volume was performed by using Simpson method. LA volume singnificantly decreased: 55.35 + 8.59 ml at baseline, 60.88 + 8.99 ml after 3 month and 67.17+ 3.21 after one year (p – 0.005). Byusing STI technique globalLAstrainandstrain rate were calculated from twelveLAsegments. Paced individuals demonstrated increased global strain from 18.27+1,99%to 25.79+ 4.67% after three month but result was not significant ( p – 0.096 ). Global left atrial strain after one year remained unchanged – 22.98 + 3.33% LA strain rate was evaluated by measuring three peaks: S, E and A. Patients with DDD pacemaker revealed significant difference only in A peak measures during follow-up after three month period witch represents contractile LA function, but after one year followup S and E peaks decreased (data represented in Table 1). Data was compared with age and sex matched healthy control individuals – no singnificant difference was detected. Conclusions: Our data implicates that conventional dual chamber pacing impacts LA function due to LA overload, therefore induces LA dilation and increases contactility.
