OP04 .01: Cardiac conduction in early onset small for gestational age and growth restricted fetuses

It is known that cardiac function is altered in fetal growth restriction. To determine whether cardiac conduction is any different in appropriately grown (AGA), small for gestational age (SGA) and intrauterine growth restricted (IUGR) fetuses, we investigated the relationship between the mechanical P-R interval, fetal arterial and venous Doppler parameters. The mechanical P-R interval, umbilical artery, middle cerebral artery and Ductus venosus Doppler PI were measured for women between 24 and 32 weeks with AGA, SGA (AC <10th percentile) and IUGR (AC <10th percentile and umbilical PI>95th percentile) fetuses. The relationship between cardiovascular parameters and gestation was defined and Doppler values converted to Z-scores in relation to gestational age. 48 AGA, 11 SGA and12 IUGR cases were included. The mechanical P-R interval was no different between SGA (128.7, 95% CI 121.5-137.0) and IUGR (122.0, 95% CI 113.4-128.9) when compared to AGA fetuses (126.8, 95% CI 124.3-130.2) p = 0.7. No relationship was found between P-R interval with gestational age or between arterial and venous Doppler Z-score and mechanical P-R interval in SGA or IUGR fetuses. SGA and IUGR fetuses demonstrate altered cardiac function but normal conduction in the late second and early third trimester of pregnancy.
