Aims & Objectives: Continuous Renal Replacement Therapy (CRRT) is utilized in four specialty intensive care units (ICUs) within the hospital. A multidisciplinary CRRT review committee was initiated to discuss morbidity and mortality related to CRRT use and any patient/system issues. Our aim was to address concerns regarding CRRT, identify educational needs, and ensure consistency in practice. Methods: Data was collected from the cardiac, medical, medical/surgical, and neonatal ICUs in a free standing academic children’s hospital in an urban setting. Formalized CRRT review began in 2009, with data collection being achieved in all four ICU’s by 2012. The same data collection tool captured information on all patients receiving CRRT. Regarding education, every staff nurse operating CRRT completes an online competency and annual review. CRRT simulations and clinical and problem solving sessions are offered at regular intervals. Results: Overall, 17,298 CRRT hours occurred from 2012-2018. During that time, unscheduled circuit changes decreased from an average of 1% in the first three years to an average of 0.5% in the most recent three years. Over that time period, a number of concerns were brought to the multidisciplinary CRRT review committee that resolved with a change in practice.Conclusions: The number of unscheduled circuit changes decreased after the creation of the multidisciplinary review committee, which relays communication and education to all four ICUs. This committee’s work has shown that it is possible to have CRRT run successfully in different ICUs, and allows patients to be cared for in their unique ICU homes.
