“How could I apply this information?” While there are positive results for the use of transcranial direct current stimulation (tDCS) in adult populations, its use in both adults and children is largely experimental and clinical relevance has not been reliably demonstrated. Overall, there is weak positive evidence for its application in pediatric cerebral palsy, primarily as an adjunct therapy. This preliminary study's data suggest that cathodal stimulation during bimanual training does not significantly alter cardiovascular/autonomic function. Higher-powered, randomized, controlled trials are needed to demonstrate the efficacy and safety in pediatrics. The authors recognize that future research is needed to address cathodal and anodal stimuli across various cortical regions to more accurately portray the autonomic impacts. From a parental perspective, this study may alleviate some concerns related to the cardiovascular effects of cathodal tDCS if considering enrollment in clinical trials. “What should I be mindful about when applying this information?” The population that this information was applied to encompassed a limited group of 8 participants and a specific application type. It was conducted with an open-label design, without a control group. Cardiovascular function was tested pre- and postintervention, with no data collected during the intervention, and there was heterogeneity in the lesion location and size. The preliminary findings represent a minute population when looking at clinical applicability, in terms of participants and intervention. We must continue to be mindful of the potential cardiovascular impacts as different parameters and types of stimulation may produce varying outcomes. It would also be wise to bear in mind the expected impact of therapy on cardiovascular function if applying tDCS as an adjunct. The current study investigated tDCS when applied during bimanual training; effects combined with more strenuous activities, such as treadmill training, should also be considered.
