Abstract Background Tumor Necrosis Factor (TNF) is the master regulator of chronic inflammation and immune dysfunction, which play a critical role in Alzheimer's Disease (AD). The current TNF inhibitors are contraindicated for neurologic disease due to complications (i.e., cause demyelination). XPro1595 is a second‐generation inhibitor designed to overcome the limitations of the current generation of TNF inhibitors to treat neurologic disease. We have received the Part the Cloud to RESCUE Award from the Alzheimer’s Association to conduct a biomarker directed, proof of biology, Phase 1b study. Method 18 patients with a diagnosis of Alzheimer’s disease and biomarkers of inflammation in the blood were eligible to participated in a 12‐week, dose escalation study to establish safety and proof of concept that XPro1595 can reduce inflammation within the CNS. Result This presentation will focus on the challenges and lessons of using inflammatory biomarkers for patient enrollment in AD studies along with the safety data and available efficacy data. Conclusion Clinical data will be presented.
