Bronchiectasis in Italy: data from the national registry IRIDE

There are limited reported data on bronchiectasis characteristics and management in Southern Europe, particularly in Italy. A secondary analysis of the Italian Bronchiectasis Registry (IRIDE) was conducted. Adults with non-CF bronchiectasis were enrolled across 18 Italian sites between 2015 and 2019. Among the 1,098 patients (mean age: 63.5 years; 67% female) enrolled, the most frequent aetiologies were idiopathic (48%), post-infective (16%), COPD (9.9%) and immunodeficiency (4.9%). Co-existing asthma was present in 21% of the cohort. Using the Bronchiectasis Severity Index, 33% were classified as mild, 37% moderate and 30% severe. The mean exacerbation rate at baseline was 2.2/year. The most frequent pathogens giving chronic infection were P. aeruginosa (19%), H. influenzae (6%) and S. aureus (4%). The mean FEV1%pred was 81%, and 69% of patients were treated with inhaled corticosteroids with and without bronchodilators. Mucoactive drugs and macrolides were used by 7% of patients, while inhaled antibiotics were rarely used (1.5%). Chest physiotherapy was practiced by only 42% of participants at baseline. Using previous published quality standards for bronchiectasis, including testing for immunoglobulins (56%), ABPA (50%), use of chest physiotherapy (42%), P. aeruginosa eradication for new isolates (25%), bronchodilators (62%) and pulmonary rehabilitation (21%) for patients with significant breathlessness, and prophylactic antibiotics for frequent exacerbators (11%), none exceeded the recommended 80% target. Italian patients with bronchiectasis are predominantly idiopathic with a high rate of P. aeruginosa infection. Quality standards for bronchiectasis management are not met across Italy.
