Intracerebral abscess after adenotonsillectomy

Introduction The adenoctomy with tonsillectomy is a common surgery in the ENT-Department. An intracerebral abscess after this surgery is a very rare complication. This complication is life threatening for the patient and needs to be treated rapidly. Case presentation A 4-year old girl was referred to us by a resident ENT-Specialist. Five days before, she underwent an adenotonsillectomy. On the second day after surgery she had a swelling of the left eyelid; because of the increasing swelling, she was examined by an ophthalmologist. He prescribed her antibiotic eyedrops. Under this therapy, the swelling increased and the child developed fever. She was then examined by another ophthalmologist, who prescribed her an oral antibiotic. On the fith day after the surgery she came to our ambulance. Diagnostic Investigation On the first visit, the child was somnolent and had a phlegmon of the eyelid. Because of the reduced general condition, we immediately carried out a CT-Scan of the brain and the paranasal sinuses. This showed multiple intracerebral abcesses and inflammatory changes of the left orbita. Therapy On the same day, the colleges from the neurosurgy drained one abscess to get a microbe collection. In the microbiological examination they could detect Streptococcus intermedius. The child began an antibiotic treatment. Under the treatment, the patient underwent control imaging (MRT).
