Comment on: Guidelines for treatment of umbilical and epigastric hernias from the European Hernia Society and Americas Hernia Society

Editor Henry R. Fruchaud said in 1956: ‘Surgical treatment of inguinal or femoral hernias should not be the simple closure of the inguinal canal or femoral ring, but deep abdominal wall reconstruction in the groin region’1. Considering Fruchaud's vision, it may be not a surprise that in the recent guidelines2, ‘It is suggested that a flat permanent mesh is placed in the preperitoneal space for open umbilical or epigastric hernia repair’. However, we think that when ‘tailoring the approach…’ technical considerations must be discussed. Adipose tissue may be inside the fascia defect and should be excised; suturing the peritoneum is essential; preperitoneal space dissection should be meticulous and gentle; drainage is not essential. Finally, we congratulate all the authors and request that the next guidelines are even more global.
