Postpartum hemorrhage emergency response using a bundle approach

Each year, approximately 2.8 million women lose their lives or become injured from pregnancy related causes worldwide. Despite global efforts, postpartum hemorrhage (PPH) remains the leading cause of maternal mortality and morbidity in LMICs. Suboptimal care contributes to poor maternal health outcomes; however the majority of PPH-related deaths are preventable through timely interventions and have the potential to be averted by quality of care improvements. In recent years, care bundles have been used to deliver high quality clinical care by promoting compliance with select best practices and process improvement, and are associated with improved adherence to clinical guidelines and better patient outcomes. Building on 10 years of innovative work to reduce PPH-related death and disability, the Massachusetts General Hospital Division of Global Health Innovation has been collaborating with senior Ob/Gyns and maternal health champions in India and Kenya to co-create a comprehensive training package for delivering quality PPH Emergency Response using a bundle approach. Based on WHO guidelines for PPH management, a holistic, cost-effective, and low-technology package has been developed in an iterative fashion with in-country leaders. The focus of the program is not only on strengthening the clinical emergency response to PPH, but also on a comprehensive implementation approach to support health systems strengthening and local capacity building to improve PPH management at the facility level and across the healthcare network. The 3-day interactive, simulation-based program trains clinicians on rapid, quality emergency response and respectful maternity care, and facility managers/quality officers/government-level supervisors on the following non-clinical elements to ensure successful implementation of the program. Teamwork and Communication Facility Readiness Network Integration Data, Monitoring and Quality Improvement Leadership The PPH Emergency Response program includes slide presentations, trainer’s manual, how-to toolkits, checklists, post-training technical assistance, monitoring and evaluation support, and lays the groundwork for national policy integration. It is designed to be flexible and adaptable to different contexts and countries based on their specific needs. Strong emphasis is given to utilizing and enhancing local mentorship and quality improvement frameworks for program sustainability. To date, the program has been piloted by in-country champions in over 50 facilities in India and Kenya.
