Purpose – Patients that re-visit after hospital discharge represent a high-risk population that are considered a surrogate metric for quality of care. They are associated with higher cost, greater resource utilization with potential for claims denials. Since the inception of financial disincentives by the Center for Medicare and Medicaid Services, hospitals have focused on reducing 30-day re-admissions.
